RULES GOVERNING USE OF BUILDING FACILITIES

FORM SS-6-2005

In endeavoring to serve the community organizations and individuals desiring to use the facilities of the Saint Sarkis Armenian Church for their functions, the Parish Council has set forth the following regulations in the interest of the efficient operations 6f its buildings and protection of its property.

1. The entire premises are smoke free.

2. Requests for the use of facilities are to be made in writing on FORM SS-6/05 with details

as to the nature of the affair, the name of the renter and the number of people expected.

All required arrangements must be cleared with the rental manager.

3. No food or drink of any kind may be served in any place other than the facility contracted.

The serving of food must be handled by the applicant, who must supply all necessary equipment and be
responsible for leaving the premises in good order.

a) No keg beer permitted at any time in any facility

4. All exit doors must be left free and unobstructed. No tables or decorations shall be placed in front of any

door or exit.

5. All lighting and stage equipment must be set with the permission and approval of the Church office and

under its supervision, and may be used only when rental manager and/or the Church office approves the

installation.

6. No decorations may be used which require pasting or fastening on walls or doors by any means such as

scotch tape, stickers, tacks etc. Details at rental office.

7. Permission to bring in special equipment for any function must be obtained from the rental manager and/or the Church office and such equipment must be removed immediately after the function.

8. The Church assumes no liability or responsibility for damage or loss of personal property of any kind and all such personal property, including motor vehicles, brought or left on the premises shall at all times be at the owner’s risk. Attendants for check room and parking are the responsibility of the renter.

9. No equipment of the Church is to be removed from the premises at any time by any individual or

organization.

10. The piano must not be moved without the express permission of the rental manager and/or the
Church office.

11. The applicant using Church facilities shall be held responsible for loss of equipment, breakage, damage

to the equipment and physical property and will be billed for any such loss, breakage and damage.

12 Publicity material or printed matter which may affect the Church or its facilities must be submitted to
the rental manager and/or the Church office for clearance in advance of its release.

13. Proper decorum and a reasonable standard of behavior shall be maintained by all persons and

organizations using Church facilities.

14 The Renter agrees that it shall not permit the premises to be used for any illegal, obscene or immoral purposes or bring or permit any obscene, illegal or immoral material on the premises and shall not permit any obscene, lewd, nude or semi-nude live. The Renter agrees that it will not permit any conduct on the premises, which would be objectionable in a complex housing a religious institution.

15 All laws and regulations, particularly those with reference to safety and/or fires established by the City of New York shall be strictly observed at all times.

16 The Refundable Security Deposit is required at time of reservation:

a) shall be forfeited by the Church member applicant if subsequent facts determine the use of hall was not

for the specific close relative (wife, husband,. daughter, son, mother, father) stated in application/contract form

b)         shall be refunded to the applicant within ten days after date of event. However, should Administrator's
            inspection of facilities determine damages or negligence, the deposit will be used toward remedying the
           condition and the applicant will be billed for the amount in excess of the deposit.

c) In addition, any custodial overtime required to restore the church facility to its normal use will be deducted at the rate of $25 per hour. This includes cleaning, dish washing furniture rearrangement and the removal of decorations.

d) Shall be forfeited if event is cancelled two weeks prior to reserved date

e) However, upon a showing of good cause for the cancellation, the sum so paid may be credited to applicant as part payment of the security deposit to reserve the facility for a mutually agreeable future date within one (1) year of the date originally reserved If the facility is not reserved by the applicant within the time limit, the Church shall retain the sum so paid as liquidated damages.

17. Balance of rental fee is payable on day of event.
